Active Care Network exists to look at areas of transport need and where appropriate provide flexible, special care transport assistance, within the available resources, for eligible "transport disadvantaged" residents of the Blue Mountains and Penrith Local Government Areas.
Who is Eligible?
Active Care Network provides Special Care Small Group Transport for the Home and Community Care (HACC) target group such as the frail aged and people with disabilities. Our service extends to residents of the Blue Mountains and Penrith Local Government Areas.Do you need help getting to:
- Shopping
- Social outings
- Health & medical appointments
- Respite
- Centre based meals
- Oncology treatments
We provide transport assistance to people of all ages, including the frail, elderly, people with disabilities - young or old, or with special needs, including wheelchairs and their carers. Also some short term assistance to health related appointments is available for people who are temporarily "transport disadvantaged".
Our service extends to residents of the Blue Mountains and Penrith Local Government Areas. We can help you.
